 癌は、遺伝子の異常が蓄積することにより、発生、進展していくことが明らかになってきました。遺伝子の異常により、抗癌剤や放射線治療などに対する感受性が異なることも明らかになってきました。近年、これらの異常な遺伝子を標的とする分子標的治療の発展はめざましく、臨床応用により患者さんのQOLの向上などに貢献しております。第一内科では、総合情報センターの協力のもとに北海道広域医療ネットワークを利用したDNAアレイ遠隔遺伝子診断の開発および実証実験を行い、地域に関係なく住民が最新・最適医療を受けることができる可能性を示してきました。現在、医療機関では血液等を採取するだけの新診断システムを構築しつつあり、患者さんだけでなく、疾病感受性予測などにより、地域の健康や福祉へのさらなる貢献をめざしております。

We have been engaged in over 30-year community-based study aiming to determine epidemiological and clinical aspects of diabetes and cardiovascular disease among the Japanese population. The Tanno and Sobetsu study was a rare example of a Japanese epidemiological study done by an internal medicine department. The Tanno-Sobetsu study was of two towns in northeastern Hokkaido, Japan and began in 1976. The national census at that time showed a population of 5,568 in Tanno (2,732 men, 2,836 women), and 4,447 in Sobetsu (2,156 men, 2,291 women). Both towns were agriculturally based, with Tanno involved in field cropping and Sobetsu in fruit cultivation.

The number of breast cancer cases in this country is in a tendency to increase, and approximately 2,500 female in Hokkaido are diagnosed as breast cancer a year. We try for the discovery of the early-stage breast cancer (Stage 0 orT) by palpation, mammography and ultrasonography, aiming at a fall of the breast cancer mortality. For the purpose, breast specialists from our department perform breast cancer examination at many cities in Hokkaido. Also, we work on enlightenment of the breast cancer examination through "Pink ribbon festival in Sapporo".

Many people from children to eldely people enjoy various sports activities. However, inappropriate activities may cause sports injuries. Our department of orthopaedic surgery has sports clinic for people who suffer sports related injuries. This system of sports clinic can improve results of treatment by our specialized medical staffs. We develop this system and open similar clinics at hospitals in Sapporo or some cities in Hokkaido. We believe it can contribute prevention and better treatment of sports injuries of sports athletes. We also give medical supports to winter sports athletes in Hokkaido such as skaters and skiers in cooperation with Japan Olympic Committee.

There are few studies on the effective curriculum development of required community medicine clerkship for fostering a community oriented general practitioner in Japan. Our department is trying to create the evidence of it. We have been introducing portfolio-based learning in the clerkship. Students need to include daily reflection, significant event analysis, community diagnosis, life-history research and mini-research on community health problem to their portfolio. We are evaluating the effect of portfolio-based learning concerning to students’ capability of understanding of patients’ family and community. Our study is revealing the importance of portfolio-based learning including multiple educational strategies and reflective learning for fostering community orientation of medical students.

It is requested from the community health practice to understand medical needs, to contemplate the health care of the entire community, and to act when engaged in the community health as a family doctor and a general practitioner. The understand of the health problem by the Delphi method is useful as one of the techniques. We apply this method practice, execute the research of the establishment of the method of the empowerment of the community by the result of obtaining, and have gotten the result accumulated. We are now planning to do an action research using Delphi method in several community medicine practice sites in Hokkaido.

 広大な医療圏を抱える北海道においては、医師の偏在が地域の医療過疎を増大させ、過疎地に暮らす人々に不安を与えるなど、社会問題となっている。
 この取り組みは、大学附属病院の教育機能を活用し、医師が地域に定着するシステムを確立することにより、こうした課題の解決に寄与しようとするものであり、特に地域医療の支援に従事する大学病院からの医師のスキル向上と不安解消に重点を置いたプログラムとした。すなわち、心肺蘇生や患者搬送などの救急医療の実践教育を実施する「緊急時対応能力養成コース」、勤務先地域の特性と医師の専門性を踏まえたテーラーメイドの集中教育を実施する「モジュール別コース」など事前トレーニングを後期臨床研修と一体で行い、着任後はITを活用したテレビ会議システムや医療情報の提供を行うサポート体制を整備し、地域のニーズに合ったプライマリ・ケア医の着実な要請を目指すものである。

 空知郡南幌町の40歳以上全住民を対象とした「生活習慣と健康」に関する疫学調査を通じて,身体的,精神的健康の向上に貢献する生活内容および医療費と生活習慣の関係を解析し,EBMに基づく,健康づくりの方策を提案している。現在は主に高齢者のADL維持に貢献する具体的な生活内容を探索し、この成果を生かし、住民データベースの構築と保健師による高齢住民の健康指導に役立てている。

Since 1998, the number of suicides in Japan has surpassed 30,000 every year. In 1996, we organized a support group for bereaved families who had lost family members to suicide. Though it was originally a support group, it has now also developed into a self-help group. As a general rule a meeting is held once a month at the university. Members talk about their current state of mind, as well as their thoughts about the suicide and the deceased in the meeting. The group is managed by 2 female psychologists, a nurse, and secretariats chosen from the concerned parties.

Under the difficult environment of child-care, to talk slower and examine your own child-care such as food, exercise and daily rhythms brings parents confidence and relief toward child-care. Parents, who bear child overseas, in the same way, need the opportunity of a constant counsel where they are able to express their anxiousness and what they are worried about their child-care in Japanese.

 わが国の高齢者の20%前後は少なくとも年に1回の転倒を経験するとされています。転倒を契機に発生するケガや転倒恐怖感は、その後の日常生活活動に制限を与え、寝たきり・要介護状態を招いてしまうことが少なくありません。したがって、いつまでも健やかでいきいきとした生活を送るために転倒を予防することが大切になります。現在、北海道の地域特性を踏まえた転倒予防対策をいかに地域と連携をして展開していけばよいか検討を進めています。

 障害をもつ人が治療的リハビリテーションを終え、地域に戻り生活を再構築しながら社会参加を目指していくためには総合的な支援体制づくりが必要である。その実現のためには地域リハビリテーションの実践拠点施設の整備が重要となる。われわれは在宅障害者を対象に生活適応能力に主眼をおいたサークル活動を実施してきた。障害者が自己の能力で活動をやり遂げる力を獲得すること(生活力の向上)は、家庭さらには社会においての役割を確立することに繋がり、本人の生活への満足度も格段の向上を示すものと考える。

We, Integrative Palliative Care Education and Practice, have opened counseling salon for patients and their family to care their mental health and mental recovery as a part of our distribution activities. We have aimed to reassure out patients and maintain their good mental health by encouraging them to air out their struggles in facing disease while we listen to complicated feelings their family, friends and lover hold inside. There are always 2 staffs at the salon and Doctors give counsel when they are needed. Also this counseling salon functions to make us recognize an importance of medical services to maintain a good mental health for patients and their family.
